People ask what to do before a rollout. Before, during, or eight months after, the answer is the same: go and watch. Not a survey, which measures what people are willing to say about themselves, but four hours of sitting beside people while they work.
Three questions on one page, filled in per desk:
- What did they open, and what did they do first?
- Where did they stop, and what did they do instead?
- What did they not check, and would it have mattered?
The third question is the one that changes budgets. In most teams the unchecked output is small, frequent, and invisible — and it is where the next incident comes from.
